This form is only for changing the number of classes in an active recurring FS8 membership (for example, moving between the 4, 8, and 12 times per month options).
Submission of this form does not automatically change your membership. All requests are reviewed by your studio team. Membership changes can only be submitted twice per year and cannot be made during a contract period.
